Postpartum Support Contract

I, the parent, have discussed and reviewed my wishes for care to be provided by Doulas of Central Maryland after the birth of my child, and agree to as follows:

  1. The care provided by the doula after the birth of the child shall include, but is not limited to, infant care; sibling transition assistance; breast feeding, bottle feeding and/or pumping support; postpartum physical and emotional support; errands; light housekeeping; laundry; and meal preparation.

  2. In the event that the doula works a scheduled shift between 10 pm and 6 am, the doula is permitted to sleep when the baby/babies is (are) asleep and all duties have been completed.

  3. Support services will be provided at $30.00 per hour with a minimum of 12 hours totaling $360.  A non-refundable deposit of $360 is required at the signing of this contract. The client will be billed for all contracted hours as necessary, utilizing the deposit to fulfill the final contracted hours.

  4. The doula shall provide at least 4 hours of support during the day (7 am to 9 pm).  If the doula is staying for an overnight shift, there is an 10 hour shift minimum between the hours of 9 pm and 6 am.

  5. All contracted hours must be used within 6 weeks of date of commencement unless alternative arrangements are made prior.

  6. Doulas of Central Maryland regrets that sometimes the doula will not be able to provide post care services as scheduled, due to unforeseen circumstances (i.e. family emergency, illness, extreme weather or labor support).  Doulas of Central Maryland will make all attempts to either schedule an alternate doula or to reschedule the affected shift.

  7. The focus of a postpartum doula is on baby and family's early adjustment to returning home, and therefore postpartum needs are greatest during the first eight weeks after birth. For this reason, Doulas of Central Maryland seeks to prioritize families who have most recently birthed babies.

  8. Failure by the client to amend or cancel the contracted and scheduled hours 24 hours prior to the scheduled services will result in being billed for the requested hours. If support is required with less than 24 hours notice, there will be a 1 hour ($30) convenience fee assessed.

  9. There is also a $5 per shift travel charge will be incurred for shifts outside of Carroll County.  This will be reflected in your invoice.

  10. Clients will be billed weekly on Mondays for shifts which occurred the previous Monday through Sunday (including Sunday/Monday overnight).

We, the undersigned have read this contract for postpartum doula support services.  We accept and agree to the terms and conditions.
